Responsibilities

- Assess and evaluate patient needs, establishing a personalized Plan of Care
- Perform initial OASIS assessments and ongoing re-evaluations
- Ensure patient safety through thoughtful care planning and resource coordination
- Collaborate with physicians, supervisors, and interdisciplinary team members
- Implement and revise Plans of Care based on patient progress
- Provide direct speech-language pathology services according to clinical standards
- Select therapeutic materials tailored to patient needs
- Educate and support patients and caregivers on home therapy techniques
- Maintain accurate, timely documentation and communication
- Work in a dynamic, physically demanding environment with flexibility for irregular hours
Requirements
- Master’s degree from an accredited program in Speech-Language Pathology
- Current, unrestricted SLP license in Indiana
- Minimum 1 year of clinical SLP experience
- Ability to travel 50% within your service area
- Reliable vehicle, valid driver’s license, and auto insurance
- Ability to lift 50–100 lbs. and assist with patient transfers
