About the role
- Psychiatric Rehabilitation Program Counselor assisting individuals with mental illness at August Rose Health Center. Providing support and coordinating services for improved independence and emotional well-being.
Responsibilities
- Assist individuals with mental illness to reach a higher level of independence
- Provide support with accessing and coordinating benefits, services, and community-based resources
- Facilitate services at a minimum of 4 contacts (for children & adolescents) per month
- Document all contact, interaction, and interventions including dates, locations, and types of contact
- Summarize the client’s progress monthly regarding goal progress, regression, and other achievements
Requirements
- CPR/First Aid certification
- One year or more of experience working with children and adults in a community setting
- Excellent written and oral communication skills
- Strong time-management and organizational skills
- Computer literacy
- Knowledge of community resources
- Ability to work independently and on a team
- Reliable transportation and a valid driver's license
