Requirements
Completed vocational training or degree in healthcare or social services
Ideally additional training/certification as a Case Manager
Knowledge of transition processes from hospital to long-term care facilities
Strong professional and interpersonal skills for interdisciplinary collaboration
Organizational and structural skills for systematic planning and efficient coordination
